Services to fulfill all needs

We at DCP provide our clients a variety of homemaker personal care services. Through the programs we offer, DCP hopes to empower every client and help them form community relationships that support client self-sufficiency. Each service is described in this section.

Supported Living
Supported Living is a program that aids individuals with MRDD in living independently in their own communities with public funding support. The services can be provided either at the individual’s own home or the family home where they live. An Individual Service Plan (ISP) is used to identify services and supports.
Non Medical transportation

Non-Medical Transportation is When a client uses transportation to receive Adult Day Support, Vocational Habilitation, Supported Employment Enclave, and/or Supported Employment Community Services as outlined in their Individual Service Plans (ISPs),

Residential Respite

Residential respite: the short-term care and support given to an individual due to the absence or need for relief from those who regularly provide care.

Family Homes

Family Homes are homes where shoppers live with their supplier whereas getting a few or all of the HPC administrations. All HPC administrations are indicated within the client’s Individual Service Plan (ISP).

Homemaker Person Care (HPC)

Homemaker/Personal Care, often called HPC, supports a person to be more independent while meeting their daily living needs. Homemaker/personal care advances the individual’s independence within his/her home and community and helps the individual meet daily living needs.
